What MTHFR Testing Actually Tells You — And What It Doesn't

I get this question more than almost any other in wellness consults: "I heard about MTHFR — should I get tested?" The conversation that follows is almost always more useful than the test result itself. That's not a knock on the test. It's a sign that MTHFR testing, like most genetic information, is only as valuable as the context around it. So here's what MTHFR testing in Yuma actually looks like — what it can reasonably inform, and where its limits are.

What Methylation Is — Before We Talk About the Gene

Methylation is one of the most fundamental biochemical processes happening in your body right now. It's not exotic. It's not a wellness trend. It's a chemical reaction your cells perform millions of times a day — switching genes on and off, helping process certain neurotransmitters, supporting how your body handles specific nutrients.

The MTHFR gene provides instructions for making an enzyme called methylenetetrahydrofolate reductase. That enzyme plays a key role in converting folate — the B vitamin found in food and supplements — into a form your body can actually use: 5-methyltetrahydrofolate (5-MTHF). When the MTHFR gene has a variant that reduces enzyme activity, folate conversion becomes less efficient. Not impossible. Just less efficient.

According to the NIH's MedlinePlus entry on the MTHFR gene, two variants are particularly well-studied: C677T and A1298C. Variants in one copy of the gene (heterozygous) typically produce a milder reduction in enzyme activity. Variants in both copies (homozygous) can reduce activity more significantly — though how much, and whether it matters clinically, varies considerably between individuals.

This is where the context matters.

What the Test Actually Measures

What MTHFR Testing Is Not

MTHFR variants have been discussed in connection with a wide range of health topics — cardiovascular concerns, neurological health, mood, and more. Some of that research is genuinely interesting. Some of it is still early, contested, or misapplied in ways that overstate what a genetic variant alone can tell you.

The American College of Medical Genetics and Genomics (ACMG) has published position guidance cautioning against broad clinical use of MTHFR testing in the absence of specific clinical indications. Their position, in plain terms, is that population-level MTHFR testing for general wellness purposes is not supported by current evidence as a diagnostic tool.

One More Thing Worth Saying

MTHFR has had a moment in functional wellness culture — sometimes a very loud one. The conversation online ranges from grounded to genuinely overblown, and it can be hard to sort out which is which without a clinical lens.

What I can tell you is that the variant is real, the enzyme function question is real, and the nutritional guidance implications are real — within their scope. What's not real is the idea that a single genetic variant explains everything, or that knowing your MTHFR status unlocks some hidden answer to why you feel the way you feel.

Genetics is a starting point for a conversation. It's one of the most interesting starting points we have. But the conversation is what matters.
